More than half of patients with testicular cancer treated with first-line cisplatin-based chemotherapy in a recent study developed hearing loss (HL) or tinnitus. The findings underscore the need for routine assessment for hearing loss and tinnitus during follow-up of cisplatin-treated survivors, the authors of the study emphasize. "The sudden development of hearing loss can be devastating, and often more consequential than the slow progression of age-related hearing loss," they point out. Administration of specific hearing loss questionnaires should be considered in patients who develop HL or tinnitus. These findings indicate that greater hearing loss was associated with lower odds of better overall health, they noted.

Lower PSA Screening Rates Linked to Rise in Mets Prostate Cancer

SAN ANTONIO — Veterans Administration (VA) medical centers with lower prostate-specific antigen (PSA) screening rates reported higher rates of metastatic prostate cancer, a recent retrospective analysis found. PSA screening became widespread in the 1990s, but that changed after several randomized controlled trials failed to show a consistent clinical benefit for PSA screening. They included yearly within-facility PSA screening rates as well as rates of missed screening appointments. Facilities with higher yearly PSA screening rates had lower subsequent rates of metastatic prostate cancer. Conversely, facilities with lower yearly screening rates had higher subsequent rates of metastatic prostate cancer 5 years later (IRR, 1.10; P < .001).

While healthcare systems in the United States and the United Kingdom vary dramatically, doctors on both sides of the pond experience similar stresses and responsibilities — and, on occasion, similar bad behavior. The category "sexist behavior" wasn't a choice in the US survey, but 38% of UK doctors reported having seen it. There was a smaller but still notable gap in incidents of racist language as well, 55% in the United States and 47% in the United Kingdom. Majorities in both the United States and United Kingdom agree that stress may explain bad behavior — but doesn't excuse it. Two-thirds of respondents in both the United States and United Kingdom say that doctors should be held to higher standards than the general public.
